Have the patient sign a release allowing you to share their medical details with Different Mental Health to facilitate continuity of care.
Send necessary patient information
Gather and send any necessary patient history, records, files and intake forms to Different Mental Health to complete the referral.
We reach out and schedule
Our next step is to reach out to your patient and schedule a consultation to establish care.

By submitting this referral request, you confirm that the information provided is accurate to the best of your knowledge. You also confirm that you are a healthcare provider or represent a healthcare organization legally authorized to treat the referred patient.
